- You must submit an application form (PDF, 122KB) at a service centre, by email to operator.accreditation@stategrowth.tas.gov.au or by post to GPO Box 1242 Hobart 7000.
- The replacement plate costs $30.60.
- If the number plate is lost or stolen, you must also submit a statutory declaration (PDF, 114KB) explaining the details of the missing plate. If the plate is stolen, a police report number must be included in the declaration.
