- You must submit an application form (PDF, 122KB) and pay a $30.60 fee at a service centre.
- If the number plate is lost or stolen, you must also submit a statutory declaration (PDF, 114KB) explaining the details of the missing plate. If the plate is stolen, a police report number must be included in the declaration.
