• A statutory declaration is a written statement that a person signs and declares to be true and correct before an authorised witness.
• Statutory declarations are used for many purposes, including to verify insurance claims, to prove age or when applying for sick leave or various benefits.
• You can make a statutory declaration by completing an online form, or downloading a declaration form. You can have it witnessed by a Commissioner for Declarations.
• Some agencies or organisations may require their own statutory declaration forms to be completed for certain services, such as Transport Services.
