    About applying for an IVF or fertility support rebate

    The Affordable In Vitro Fertilisation (IVF) and Fertility Support Initiative provides financial assistance to help reduce out-of-pocket costs for Tasmanians accessing eligible fertility treatments.

    If you received eligible fertility services on or after 1 July 2026 to 30 June 2028, you may be able to claim up to $2,000. Eligible services must be provided in Tasmania by TasIVF or Fertility Tasmania.

    How to apply

    What you need to apply

    To apply online you must have:

    • an email address
    • declaration form confirming treatment from Tas IVF or Fertility Tasmania (who will provide the form)
    • proof of identity document
    • recent bank statement or account confirmation letter from your financial institution that shows your name, address and bank account details.

    To apply at a service centre you must have:

    • application form (PDF, 365KB)
    • declaration form confirming treatment from Tas IVF or Fertility Tasmania (who will provide the form)
    • proof of identity document
    • recent bank statement or account confirmation letter from your financial institution that shows your name, address and bank account details.

    Can I apply?

    You may be eligible if you meet all of the following:

    • you are a Tasmanian resident
    • you have incurred eligible treatment costs on or after 1 July 2026 to 30 June 2028
    • your treatment was provided by TasIVF or Fertility Tasmania
    • you are the person undergoing treatment with the intention of becoming pregnant.

    This initiative is available to anyone who may become pregnant, including women, transgender men and non-binary people.

    Proving your identity

    To apply you will need one of the below identity documents:

    • driver licence
    • passport
    • photo identification or personal information card
    • birth certificate (not commemorative or extract)

    If you have changed your name

    Your identity document must show your full name, including all given and family names. 

    If you need to show proof of change of name you must provide one of the following documents:

    • divorce papers issued by a court indicating the name being reverted
    • change of name certificate or deed poll certificate issued by Births, Deaths and Marriages
    • marriage certificate issued by Births Deaths and Marriages.
