- You can lodge a completed application form (PDF, 2.2MB), your certified identity documents and credit card details by email at bdm@justice.tas.gov.au.
- You can also post the completed form, certified identity documents and credit card details to Registry of Births, Deaths and Marriages, 199 Collins Street, Hobart, Tas 7000.
- It costs $237.16 to add details to a birth record.
- If you would like to order a birth certificate there is an additional fee.
