Apply to add parent's details to a birth record

• You can lodge a completed application form (PDF, 2.2MB), your certified identity documents and credit card details by email at bdm@justice.tas.gov.au.
• You can also post the completed form, certified identity documents and credit card details, cheque or money order to Births, Deaths and Marriages, 30 Gordons Hill Road, Rosny Park, TAS 7018.
• It costs $215.38 to add details to a birth record.
• If you would like to order a birth certificate there is an additional fee.
