Add marriage details to a birth record

Key info

• You can lodge an application form (PDF, 2.2MB), your certified identity documents and marriage certificate, and credit card details by email at bdm@justice.tas.gov.au.
• You can also post the completed form, certified identity documents and marriage certificate, and credit card details to Registry of Births, Deaths and Marriages, 199 Collins Street, Hobart, Tas 7000.
• It costs $237.16 to add details to a birth record. If you don't return the child's original birth certificate by post, you must pay an extra fee to order a new birth certificate.
• Both parents must sign the form in the presence of a witness over the age of 18 years.
