Add marriage details to a birth record

Key info

• You can lodge an application form (PDF, 2.2MB), your certified identity documents and marriage certificate, and credit card details by email at bdm@justice.tas.gov.au.
• You can also post the completed form, certified identity documents and marriage certificate, and credit card details, cheque or money order to Births, Deaths and Marriages, 30 Gordons Hill Road, Rosny Park, TAS 7018.
• It costs $215.38 to add details to a birth record. If you don't return the child's original birth certificate by post, you must pay an extra fee to order a new birth certificate.
• Both parents must sign the form in the presence of a witness over the age of 18 years.
