• If you have been impacted by the recent Optus cyberattack there are a number of actions you can take to protect your identity documents.
• If Optus has confirmed your Tasmanian licence has been compromised, you can apply for a new driver licence number. You must show your Optus data breach communication or provide a Statutory Declaration (PDF, 114KB) that states your personal information was breached by the Optus data leak. There will be no charge for this service.
• If Optus has confirmed your Tasmanian Government Personal Information Card has been compromised, you can apply for a replacement card which will have a new card number. You must show your Optus data breach communication or provide a Statutory Declaration that states your personal information was breached by the Optus data leak. There will be no charge for this service.
