Key info

  • Eligible Australian defence force veterans, their partner, spouse or widow, can receive two $100 vouchers per year.
  • Vouchers can be used at participating gyms, community recreation or sporting clubs, welfare activities, or recreational pursuits that requires payment or a charge to participate.
  • The voucher program runs from 1 July - 17 June each year.
  • There is no fee to apply.

    About applying for a veteran wellbeing voucher

    Voucher numbers will be emailed, posted or printed for you depending on how you apply.

    You can give your voucher numbers to the participating activity provider to use your vouchers. 

    You can search for participating activity providers online.

    Online

    To apply online you must:

    • provide an email address
    • upload proof of service and identity documents.

    Post or email

    To apply by post or email you must:

    • return the completed application form (PDF, 149KB)
    • include copies of proof of service and identity documents.

    To apply at a service centre you must:

    • return the completed application form (PDF, 149KB)
    • show original proof of service and identity documents.

      Can I apply?

      To apply for the veteran wellbeing voucher program you must be:

      • A person who is a former member of the Australian Defence Force (ADF), including a reservist, with at least one day of continuous full-time service and the holder of a card issued by the Department of Veterans' Affairs (DVA), or an ADF-issued service ID to verify eligibility.
      • The partner or spouse of a former member listed on a DVA Pensioner Concession Card (PCC), or a war widow/er with a DVA Gold Card; and
      • Living in Tasmania; and
      • Participating in an eligible gym, community or sporting club, welfare activity, or recreational pursuit that requires payment or a charge to participate.

      Proving your identity

      Identity

      You must provide evidence of your name, date of birth and address. You must provide original documents. You can provide your:

      • Tasmanian driver licence, or
      • Tasmanian personal information card.

      If you don't have a driver licence or personal information card, you can provide one document with your name and date of birth and one document with proof of your current address:

      • Australian Birth certificate (or extract)
      • Australian Citizenship papers
      • Passport
      • Change of Name or Marriage Certificate (from Births, Deaths and Marriages) to prove any name change
      • Tasmanian firearms licence
      • Australian Government issued pension card
      • Recent bank statement or utility account with current residential address.

      Proof of service

      Ex-serving members must also provide:

      • ADF Service Number
      • ADF or DVA Card

      Partners or spouses of an ex-service member must also provide:

      • DVA Pensioner Concession Card

      War Widow/er(s) must also provide:

      • DVA Gold Card
